Qualifications

• Active RN license in state of practice.
• Minimum 2 years in health care or care management.
• Valid driver’s license and reliable transportation.
• Familiarity with EMR and HIPAA requirements.
• Ability to work independently and manage time well.
• Excellent verbal and written communication skills.
• Willingness to travel 25–40% as required.

Responsabilités

• Complete comprehensive member assessments per timelines and determine eligibility for care management.
• Develop and implement care coordination plans with members and care networks.
• Conduct telephonic, home visits or in-person meetings as needed.
• Monitor care plans, document interventions and adjust goals accordingly.
• Maintain ongoing member caseload for outreach and management.
• Promote integration of behavioral health, LTSS, and community resources.
• Lead ICT meetings and collaborate with the interdisciplinary team.
• Use motivational interviewing and Molina guidance to support change.
• Identify barriers to care and provide coordination and assistance.
• May provide consultation and medication reconciliation as needed.
• Travel 25–40% may be required based on state/contractual requirements.
