Job summary

Molina Healthcare, Inc. is seeking a Registered Nurse Care Manager to support care management and care coordination across the continuum.

You will work with multidisciplinary teams to ensure member progress toward outcomes and contribute to quality and cost-effective care strategies. The role involves developing and implementing individualized care plans, conducting assessments and home visits as needed, and collaborating with caregivers, physicians, and the member support network.

Qualifications

• At least 2 years experience in health care, preferably in care management or related setting.
• RN license must be active and unrestricted in state of practice.
• Valid driver's license and reliable transportation for required travel.
• Understanding of EMR and HIPAA regulations.
• Knowledge of community resources.

Responsibilities

• Completes comprehensive member assessments within regulatory timelines to determine eligibility for care management.
• Develops and implements care coordination plans with the member and care team.
• Performs telephonic, in-person, or home visits as required.
• Monitors care plan effectiveness, documents interventions, and adjusts as needed.
• Maintains ongoing member caseload and outreach for care management.
• Facilitates integration of behavioral health, LTSS, and community resources.
• May provide ICT meeting coordination and peer consults.
