Job summary

Molina Healthcare, Inc. in Florida seeks an experienced RN to join our care management team, coordinating member care across the continuum to improve outcomes and cost efficiency.

As part of a multidisciplinary team, you will assess, plan, and monitor care, conducting visits as needed and supporting members and care networks. Travel 25-40% may be required; active RN license and 2+ years of healthcare experience are needed.

Qualifications

• At least 2 years experience in health care, preferably in care management, or equivalent.
• Registered Nurse (RN) with active, unrestricted license.
• Valid driver's license and reliable transportation for travel.
• Understanding of EMR and HIPAA.
• Knowledge of community resources.
• Ability to work independently and with diverse populations.
• Strong communication and time-management skills.
• Proficiency in Microsoft Office and online portals.
• Preferred CCM certification.

Responsibilities

• Completes comprehensive assessments of members per timelines and identifies care management qualifiers.
• Develops and implements coordinated care plans with members and care teams.
• Delivers telephonic, in-person, or home visits as required.
• Monitors care plan effectiveness and documents interventions and goal achievement.
• Maintains ongoing member caseload and regular outreach.
• Promotes integration of services across behavioral health, LTSS, and community resources.
• Facilitates ICT meetings and collaboration.
• Assesses barriers to care and provides coordination assistance.
• May provide consultation to peers.
• RNs may handle complex cases and medication reconciliation.
