Qualifications

• Certified RN in Illinois
• BLS within 30 days per AHA
• College Diploma: Nursing

Responsibilities

• Expedites consultation between referring providers and receiving physicians.
• Coordinates acceptance, admission, and transfer communications.
• Uses scripts and algorithms to facilitate transitions into Carle Hospital.
• Maintains professional relationships with referring facilities to ensure timely, safe transfers.
• Documents data in Epic and Forefront and ensures post-transfer follow-up.
