Job summary

Abrazo Community Health Network seeks an experienced RN to coordinate Transfer Center transfers from initial call to patient discharge and follow-up. You will ensure high-quality service for physicians, nurses, patients, and other professionals.

Responsibilities include coordinating referrals, outlining care plans to receiving nurses, and supporting educational initiatives for nursing staff. Strong communication and clinical expertise are essential.

Qualifications

• Education: Associates Degree in Nursing, or equivalent, from an accredited program.
• Experience: 4 years clinical experience in an acute care setting.
• Licensure/Certifications: Current Arizona, or other compact state, Registered Nurse license; BLS.

Responsibilities

• Coordinate Transfer Center transfers from the initial call through patient discharge and follow-up.
• Contact the receiving nurse to outline the patient’s condition, medical treatment plan and orders to prepare for arrival.
• Assist with development and implementation of educational programs for nurses.
• Facilitate physician‑to‑physician communication.
• Communicate with sending facility to ensure follow-up with physician.
• Assist sending facility with transport of patient by air or ground.
• Call admitting department at receiving facility to admit patient.
