Qualifications

• Education: Associates Degree in Nursing from an accredited program.
• Experience: 4 years clinical experience in an acute care setting.
• Licensure: Current Arizona, or other compact state, Registered Nurse license; BLS.
• BSN preferred; ACLS or other advanced certifications are desirable.

Responsibilities

• Respond to all referrals to the Transfer Center program in a timely manner.
• Contact the receiving nurse to outline the patient’s condition, treatment plan and orders.
• Assist with development and implementation of educational programs for nurses.
• Facilitate physician-to-physician communication.
• Communicate with sending facility to assure follow-up with Physician accomplished.
• Assist sending facility with use of air or ground transport of patient.
• Telephone receiving facility Admitting Department to admit patient.
