Job summary

Covenant House California (CHC) is a nonprofit providing sanctuary and services to youth aged 18-24 experiencing homelessness in Los Angeles. We are seeking a Human Trafficking Case Manager to support youth via intake, assessment, ISP development, and transition planning, including housing, resources, and wellbeing.

This role emphasizes trauma-informed care and collaborative case management within CHC’s shelter and street outreach programs.

Qualifications

• Minimum of 3 years of case management experience.
• Experience with homeless or runaway youth preferred.
• Bilingual English/Spanish preferred.

Responsibilities

• Provide case management services.
• Develop and maintain Individual Service Plans (ISPs) with youth.
• Coordinate intake, assessment, and resource referrals.
• Assist with transition planning including housing and budgeting.
• Document progress, maintain HMIS/ETO records and case notes.
• Facilitate group activities and participate in meetings.
• Ensure safety, cleanliness, and adherence to program policies.
