Job summary

Greenbrook is seeking a compassionate Case Manager to lead patient care coordination at our Sugar Land clinic in Texas. You will manage scheduling, authorizations, and cross-functional collaboration to ensure smooth treatment journeys for patients receiving TMS and Spravato therapies.

The role emphasizes organized, proactive patient advocacy, with a focus on timely communications, accurate record-keeping, and adherence to treatment requirements in a fast-paced setting.

Qualifications

• High School Diploma or equivalent with 3–5 years in clinical case management or related healthcare setting.
• Experience using EHR systems and Microsoft Office (Outlook, Word, Excel).
• Strong communication, organizational, and patient service skills with ability to manage multiple priorities.

Responsibilities

• Guide patients through their treatment journey from consultation to completion.
• Coordinate appointments, provider schedules, authorizations, specialty pharmacy activities.
• Monitor patient progress, address barriers to care, and collaborate with cross-functional teams to keep plans on track.
• Maintain documentation, financial estimates, and patient records within CRM and EHR systems.
• Respond to patient inquiries with empathy, professionalism, and urgency while delivering exceptional service.
• Support high-quality patient outcomes while ensuring compliance with requirements.
