Outfield Healthcare Partners is seeking a Recreation Therapist to plan, organize, and implement a comprehensive activity and leisure program for residents. You will design therapeutic recreation services addressing social, spiritual, intellectual, emotional, educational and physical needs as part of the resident care plan.

Responsibilities include assessing residents, creating individualized plans, delivering programs, maintaining documentation and coordinating monthly activity calendars.

Responsabilidades

• Develop and implement comprehensive therapeutic recreation programs and services to meet residents' psycho-social, physical, and cognitive needs.
• Assess, select, organize and implement recreational services tailored to each resident's needs and interests.
• Develop and deliver activity programming to promote engagement in normal life activities and maintain functioning levels.
• Complete the Activities section of the MDS assessment and related care plans within required timelines.
• Plan a monthly calendar of activities and post it in the center.
• Identify resident preferences and health issues affecting participation; document findings.
• Monitor resident attendance and participation daily and identify risk factors or changes in condition.
• Coordinate community activities involving residents, staff, families and the broader community.
