Talentify is seeking a dedicated Therapeutic Recreation Specialist to plan, organize, and implement activity and leisure programs for residents per the care plan. You will assess needs, develop individualized recreation plans, and coordinate with care teams to promote meaningful engagement and well‑being.

You will document assessments, prepare monthly calendars, and support resident Council activities while ensuring safety and physician approvals where required.

Responsabilidades

• Develops and implements comprehensive therapeutic recreation programs and services to meet the psycho-social, physical, and cognitive needs of residents.
• Performs assessment, selection, organization and implementation of recreational services to meet individualized needs and interests.
• Delivers activity programming to promote engagement in normal life activities and maintain/improve functioning.
• Completes resident data in MDS-related sections and participates in care plan meetings.
• Conducts admissions interviews and documents activity plans, CAA and care plans.
• Prepares monthly activity calendars and posts them for residents.
• Identifies activity preferences from interviews and records and documents findings.
• Creates an individualized plan of care from assessed needs, strengths and preferences.
• Implements strategies to increase participation considering functional or situational limitations.
• Charts attendance and participation; monitors risk factors and changes in condition.
• Plans community activities involving residents, staff, families, and community.
• Obtains physician approval for programs and maintains precautions list.
• Provides opportunities for group and individual leisure pursuits (arts/crafts, outings, religious and educational activities).
• Supports residents' rights to self-determination and documents refusals.
• Assists residents in organizing Resident Council Meetings and coordinates a Volunteer Program.
• Participates in continuing education and departmental training; supports budget planning and monthly reporting.
