Job summary

AccentCare, Inc. is seeking a full-time Territory Patient Admissions Coordinator to manage referrals within the St. Paul area. The role requires one year of home health or hospice experience and familiarity with EMR systems, preferably HCHB.

You will work with referrals, agency staff, and sales teams to ensure timely and accurate admissions, coordinating documentation and payer information as needed to finalize quality referrals.

Qualifications

• High School or GED required.
• Minimum of one year experience in home care, hospice, or personal care services.
• EMR experience; HCHB preferred.
• Experience with Medicare/Medicaid payors preferred.

Responsibilities

• Collaborates with Referral Source, Sales teams, Clients and/or local agency operations to collect needed information to process a quality referral.
• Documents all referral information, actions taken, and follow-up required in the medical record, using the agency’s software to complete workflow tasks within established time frames.
• Ensures timely attention to referrals received from eFax, telephone, or referral portal/Hospital EMR portal.
• Reviews Eligibility Alerts and Responses for Medicare Clients when applicable.
• Requests H&P, Discharge Summary, and other documents needed for ICD 10 coding by the CPOCC team.
• Performs follow-up calls to Client/Caregiver/Referral Source to confirm information and clarify any out-of-pocket expenses.
