Job summary

VitalCaring Group is seeking an Account Manager to drive referral growth and admissions within an assigned territory. You will build and sustain relationships with physicians, hospitals, SNFs, ALFs, and other healthcare partners to expand our home health and hospice reach.

In this field-based role, you’ll conduct in-person visits, identify new revenue opportunities, and collaborate with clinical teams to ensure smooth patient transitions.

Qualifications

• Direct healthcare-related sales experience.
• Proven ability to generate referrals and admissions.
• In-person field-based selling in a defined territory.
• Ability to build and sustain referral relationships.

Responsibilities

• Develop and grow defined territory through in-person relationship building.
• Establish and maintain referral partnerships with physicians, hospitals, SNFs, ALFs, and community providers.
• Generate new business through outreach, calls, and networking.
• Drive admissions growth by meeting referral and performance targets.
• Serve as primary contact for referral partners and ensure follow-through.
• Collaborate with clinical and operational teams to support patient transitions.
