Job summary

Connectly Recruiting seeks a Clinical Manager to oversee patient care services and clinical staff in a Home Health setting across California. A valid RN license and leadership experience are required.

You will coordinate patient care, supervise staff, and ensure safe, effective care delivery. In this role, you will drive CHAP/regulatory readiness, lead QAPI initiatives, monitor patient satisfaction and complaint resolution, and support regulatory surveys with analytics.

Qualifications

• RN license in the state of operation
• 5 yrs RN experience with 2 yrs or more supervisor experience in Home Health
• Experience managing RN services in the home, community, or clinic is preferred
• Must possess and maintain CPR certification
• Must be a licensed driver with insured vehicle and ability to travel to all locations

Responsibilities

• Oversee day-to-day clinical and administrative operations and serve as primary contact for staff and patients during operating hours
• Ensure care delivery aligns with professional standards, agency policies, and state/federal regulations
• Provide leadership and backup coverage for PCM and CSS roles as needed
• Coordinate patient care including intake and referral processes with interdisciplinary team
• Lead QAPI initiatives, track incidents and trends, implement corrective actions
• Maintain survey readiness and conduct detailed audits and HHCAHPS distribution
• Oversee Emergency Preparedness Planning (EPP) and post-activation reporting
