Job summary

Acloché LLC seeks a Clinical Manager in California to oversee patient care services and clinical staff supervision, ensuring safe, effective care and regulatory readiness. You will lead CHAP readiness, QAPI initiatives, and patient satisfaction initiatives.

Responsibilities include coordinating patient care, supervising staff, managing OASIS/discharge data, and driving performance analytics across locations, with travel as needed.

Qualifications

• Graduate from an approved school of professional nursing and licensed to practice as a registered nurse in the state of operation.
• 5 yrs RN experience with 2+ years supervisor experience in Home Health.
• Experience managing RN services in the home, community, or clinic is preferred.
• Must possess and maintain CPR certification.
• Must have a valid driver’s license and be able to travel to all business locations.

Responsibilities

• Oversee day-to-day clinical and administrative office operations and serve as primary contact for staff and patients.
• Ensure regulatory compliance with professional standards, agency policies, and state/federal regulations.
• Provide leadership and backup support when needed, including covering PCM and CSS roles.
• Coordinate patient care by managing intake/referrals and collaborating with an interdisciplinary team.
• Drive quality and assessment initiatives, monitor patient needs, and collaborate on transfers/discharges.
• Manage OASIS and discharge data, including reviewing and editing forms.
• Lead QAPI initiatives, monitor incidents, analyze trends, and implement corrective actions.
• Maintain survey readiness and analytics, including HHCAHPS survey distribution within SHP.
• Oversee Emergency Preparedness Planning (EPP) with activation, reporting, and ongoing monitoring.
