Job summary

Client Growth Resources, Inc is seeking a Clinical Manager to oversee all patient care services and supervise clinical staff in California. You will coordinate patient care with an interdisciplinary team and ensure care is delivered safely and effectively.

This role emphasizes CHAP readiness, QAPI initiatives, patient satisfaction, and survey readiness, including HHCAHPS distribution and emergency preparedness. RN licensure in CA, CPR certification, and travel readiness are required.

Qualifications

• Must be a licensed RN in the state of operation (CA).
• 5 yrs RN experience with 2+ yrs supervisor experience in Home Health.
• Experience managing RN services in home, community, or clinic settings preferred.
• Must possess and maintain CPR certification.
• Must be a licensed driver with reliable vehicle and willing to travel to all locations.

Responsibilities

• Oversee day-to-day clinical and administrative operations, serving as primary contact for staff and patients during operating hours.
• Direct care delivery to align with professional standards, agency policies, and state/federal regulations.
• Act as leader in absence of Territory Administrator and provide coverage for PCM and CSS roles.
• Coordinate intake and referrals with interdisciplinary team to create individualized care plans.
• Drive QAPI initiatives, track incidents, analyze trends, and implement corrective actions.
• Review discharge OASIS data, edit/lock Discharge OASIS forms, and oversee discipline-only discharges.
• Maintain accreditation and survey readiness, conduct audits, and manage HHCAHPS survey distribution.
• Lead Emergency Preparedness Planning (EPP), activation, reporting, and ongoing monitoring.
