Job summary

Your Talent Team seeks a Clinical Manager to oversee patient care services and staff in our California home health operations. The role drives QAPI initiatives, CHAP/survey readiness, and ensures safe, effective care across the organization.

You will supervise clinical staff, lead quality improvement efforts, and coordinate interdisciplinary care plans while maintaining regulatory compliance. Ideal candidates hold a CA RN license, 5+ years RN experience with supervisory exposure, and a valid CPR

Qualifications

• RN license required in the state of operation.
• 5 years RN experience with 2+ years in a supervisory role in Home Health.
• Experience managing RN services in home, community, or clinic settings preferred.
• Licensed to practice as a registered nurse in the operating state.
• Valid CPR certification required for clinical roles.
• Must be a licensed driver with insured vehicle and willingness to travel to all business locations.

Responsibilities

• Oversee day-to-day clinical and administrative operations and serve as main contact for staff and patients during operating hours.
• Ensure care delivery aligns with professional standards, agency policies, and state/federal regulations.
• Provide leadership and backup coverage as acting leader in absence of Territory Administrator, and for PCM and CSS roles as needed.
• Coordinate patient care by managing intake and referrals and developing individualized care plans with an interdisciplinary team.
• Drive quality and assessment by evaluating care quality, addressing patient needs, and collaborating on transfers and discharges.
• Manage OASIS and discharge data, review and edit discharge forms and oversee discipline-related discharges.
• Lead QAPI initiatives, track incidents, analyze trends, and implement corrective actions.
• Maintain survey readiness and analytics, conduct audits, and manage HHCAHPS survey distribution within SHP.
• Oversee Emergency Preparedness Planning, activation, reporting, and ongoing event monitoring.
