Job summary

CAMBA, Inc. in Bronx, NY is seeking a Case Manager to support CAMBA Housing Ventures by conducting tenant intake, assessing needs, and developing individualized living plans. You will coordinate services, maintain accurate files, and prepare required reports for ongoing program oversight.

The role requires a BA or equivalent experience, SPMI experience, and adherence to HIPAA etc. This is a full-time position with a competitive benefits package and occasional extended hours.

Qualifications

• Experience working with the Seriously and Persistently Mentally Ill (SPMI) population.

Responsibilities

• Maintain professional relationships with clients and comply with HIPAA regulations.
• Review tenant eligibility documentation and maintain tenant files.
• Conduct intake and assess tenants’ needs; develop independent living plans with short- and long-term goals.
• Identify community resources, make referrals, and assist tenants in accessing services.
• Monitor progress, document via progress notes, and participate in case conferences.
