Job summary

CAMBA is seeking a dedicated Case Manager in Bronx, NY to maintain professional relationships with clients, conduct intake, and create living plans while ensuring HIPAA compliance. You will work with tenants to identify resources and support independent living goals, reporting to the Program Director. A Bachelor’s degree is required, and experience with SPMI is preferred.

Qualifications

• Bachelor’s degree required.
• Experience with Seriously and Persistently Mentally Ill (SPMI) population preferred.

Responsibilities

• Maintain professional relationships with clients and compliance with HIPAA regulations.
• Review documentation establishing tenants' eligibility for program and make file copies.
• Create and maintain tenant files.
• Conduct initial intake and assessment of tenants and families' needs and periodic re-assessments.
• Assist tenants in attaining goals by locating community resources and referrals.
