Qualifications

  • 2-3 years of case management experience.
  • LCSW preferred.
  • Knowledge of homelessness, mental illness and substance use.
  • Proficiency in Microsoft Office; strong communication and organizational skills.
  • Must have valid Illinois driver's license, insurance, and own vehicle.

Responsibilities

  • Provide intake and assessment for new tenants and develop service plans.
  • Deliver crisis intervention and after-hours support.
  • Assist with applications and advocacy for public benefits.
  • Coordinate referrals and link tenants to housing, income, and health services.
  • Maintain tenant files and enter services into HMIS and databases.
  • Attend supervision and maintain professional boundaries.
