Job summary

Connections for the Homeless in Evanston, IL is seeking a full-time case manager to provide direct services to clients, including intake, housing assessments, crisis support, and referrals. The role requires at least one year of case management experience and a bachelor’s degree in social work or related field.

You will work on-site at 1458 Chicago Ave and collaborate with staff to develop service plans, document activities in databases and spreadsheets, and participate in group facilitation.

Qualifications

• Bachelor’s degree in social work or related human services field is required or equivalent combination of education and experience.
• Minimum one year of experience providing case management services is required.
• Strong communication skills with empathy and respect for participants are essential.
• Computer competency and comfort with data entry are required.

Responsibilities

• Provide case management for clients including wholistic goal-setting and service plans.
• Perform housing eligibility assessments (VI-SPDAT) for individuals, families, and youth.
• Coordinate with health providers and refer to services as needed.
• Track information in databases and spreadsheets and maintain records.
• Facilitate groups and provide crisis support as needed.
