Job summary

HealthWest seeks a Housing Case Manager to provide comprehensive housing-related services for individuals with mental illness, developmental disabilities, or substance use disorders. Qualifications

• Bachelor degree in human services or eligible to graduate within 90 days, transcript upon request, or five years relevant experience.
• One year of experience providing supports/services to persons with mental illness, developmental disabilities, or substance use disorders; internships count if documented.
• Valid driver's license or ability to obtain within 30 days of employment.
• Reliable transportation required to perform duties across residential and community settings.

Responsibilities

• Provide and coordinate housing-related case management services.
• Assist with housing navigation, planning, and placement to promote long-term stability.
• Advocate for clients and coordinate with landlords, property managers, and housing authorities.
• Document services, monitor progress, and provide on-call crisis support as needed.
