Job summary

St. Paul’s PACE Reasner in Bankers Hill, San Diego, is seeking a licensed Occupational Therapist to join the interdisciplinary team. The OT will design and implement therapy programs to help participants maintain independence and safety in daily activities across the Day Center, PACE Center, nursing homes, and in-home settings.

This full-time temporary role runs about two months with possible extension. Candidates must have CA OT license, geriatric experience, and be able to clear background

Qualifications

• OT program design and implementation experience.
• Experience with geriatric populations.
• Licensed Occupational Therapist in California (CA).

Responsibilities

• Design and implement an occupational therapy program to help participants maintain independence and live safely.
• Conduct participant assessments and collaborate with the IDT to develop personalized care plans.
• Focus on acute restorative or maintenance levels of care and optimize function.
• Oversee execution of care plans and perform reassessments as needed.
• Work across Day Center, PACE Center, nursing homes, and in-home environments; assist in durable medical equipment management.
