Job summary

St. Paul’s PACE Reasner in Bankers Hill, San Diego, seeks a licensed Physical Therapist to design and implement a therapy program helping participants maintain independence and safe community living.

The PT conducts assessments with the interdisciplinary team (IDT), creates personalized care plans, and oversees delivery across the Day Center, PACE Center, nursing homes and in‑home settings, with ongoing reassessments and equipment management.

Qualifications

• Graduate of a PT curriculum accredited by national bodies and licensed in CA.
• Experience with geriatric populations is required.

Responsibilities

• Design and implement a physical therapy program to maintain independence.
• Conduct participant assessments and develop personalized care plans with the IDT.
• Oversee execution of plans across Day Center, PACE Center, nursing homes and in‑home settings.
• Perform reassessments as needed and optimize function.
• Assist in ordering and maintaining durable medical equipment.
