Job summary

Neighborhood Healthcare is seeking an Occupational Therapist to support seniors in a PACE setting. You will design and manage OT plans, assess mobility, and supervise staff to implement care plans, enabling greater independence for patients.

Schedule: Mon-Fri 8:00am-4:30pm with a $5,000 signing bonus. Requires CA OT license, BLS, CPR/First Aid, and experience with the frail elderly population.

Qualifications

• Valid CA Occupational Therapist license in good standing.
• One year experience with frail or elderly population.
• Current BLS certification per AHA requirements; CPR/First Aid.
• Excellent verbal and written communication skills.
• Ability to work in a multi-disciplinary team.

Responsibilities

• Develops and implements occupational therapy programs for day services.
• Performs assessments of patients to determine mobility and abilities.
• Develops individualized plans of care to promote independence.
• Monitors support staff to ensure plans are followed.
• Educates patients/caregivers on adaptive devices and self-care.
• Records treatment plans and quarterly progress notes.
• Conducts home, hospital, and site visits as indicated.
• Ensures compliance with safety and regulatory standards.
