Job summary

San Ysidro Health, located in National City, CA, seeks an Occupational Therapist to join the PACE interdisciplinary team. The OT designs and implements therapy programs to help PACE patients remain independent and live safely at home, performing assessments and creating individualized care plans with supervision for support staff.

The role requires driving to patient sites, home visits, and collaboration with CNAs/PCAs.

Qualifications

• Licensed Occupational Therapist in CA
• CA driver’s license and reliable transportation
• Proof of current vehicle insurance (if applicable)
• PAM Certification strongly preferred

Responsibilities

• Develop and implement a program of occupational therapy for patients enrolled in PACE and accessing day services.
• Perform assessments of patients to determine abilities.
• Develop and implement an individualized plan of care to maximize independence and function.
• Guide patients and caregivers in the use of adaptive devices and therapeutic/self-care activities.
• Conduct home, hospital, and other site visits as needed and indicated by IDT/Center Manager.
• Record and sign treatments and quarterly progress notes in the patient health record.
