Job summary

United Health Centers of the San Joaquin Valley seeks a dedicated Occupational Therapist to support the PACE program. You will screen and assess member functional status, develop OT plans, and implement interventions in collaboration with an interdisciplinary team.

You will supervise Rehabilitation Aides and OTA, contribute to staff education, and ensure adherence to California OT practice standards. This role emphasizes assessment, communication, and quality of care for frail elders.

Qualifications

• Bachelor of Science degree in Occupational Therapy; current CA licensure.
• Possession of a valid California driver’s license.
• Minimum of one year of experience serving adults with disabilities or frail elderly individuals; PACE experience preferred.

Responsibilities

• Screen and assess member functional status; plan and implement OT interventions.
• Collaborate with Interdisciplinary Team on care planning and service allocation.
• Provide supervision and training to Rehabilitation Aides and OTA; ensure follow-through of programs.
• Educate staff and members on OT services; communicate with care teams and families.
• Maintain licensure and ensure compliance with OT practice laws.
