Job summary

Neighborhood Healthcare in Murrieta, CA is seeking a qualified Occupational Therapist to support our PACE program, helping seniors stay independent and live safely at home.

You will design patient care plans based on mobility assessments, supervise staff, and ensure therapy goals align with CMS and DHCS requirements. A CA license and BLS certification are required; a $5,000 signing bonus is offered.

Qualifications

• Valid CA Occupational Therapist license in good standing.
• One year experience working with frail or elderly population.
• Current BLS certification; CPR and First Aid required.
• Excellent verbal and written communication skills.
• Ability to work in a multidisciplinary team.

Responsibilities

• Develops and implements occupational therapy programs for patients.
• Performs mobility assessments to determine physical abilities.
• Develops and implements individualized plans of care to maximize independence.
• Monitors qualified support staff activities to ensure plans are followed.
• Records treatment plans and progress notes in the health record.
