Job summary

myPlace Health is seeking an Occupational Therapist to deliver personalized therapy to PACE participants across care centers, homes, and virtual settings. You will partner with the interdisciplinary team to create individualized care plans that promote independence and safety for aging adults.

Responsibilities include evaluating participants, proposing therapeutic exercises, educating families, and supporting transitions between care settings.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Licensed or eligible for California OT license.
• CPR and First Aid certification required within 60 days of employment.
• NBCOT registration preferred.
• Minimum 2 years of OT experience, preferably with frail or elderly individuals.

Responsibilities

• Collaborate with the IDT to develop and review individualized care plans.
• Deliver personalized occupational therapy to PACE participants at centers, in homes, and virtually as needed.
• Assess participants and recommend therapeutic exercises to maintain or improve function.
• Educate participants and families on mobility, safety, and independence.
• Support home safety evaluations and equipment needs as part of care.
• Lead care planning with approval authority on behalf of the IDT.
• Assist with home visits and correlate with care transitions between settings.
