Job summary

Northwestern Medicine Palos Hospital is seeking a Registered Nurse for Transitional Care. You will care for patients boarding in the ED, perform assessments and med/infusion management, and collaborate with inpatient teams to support discharge planning.

Qualifications

• BSN required and Illinois RN license.
• ACLS, PALS/ENPC & TNCC within one year of hire.
• Strong interpersonal skills and collaboration abilities.
• ADN acceptable if pursuing BSN within three years.

Responsibilities

• Assess and record patient status on admission or care entry.
• Organize health data for accuracy and accessibility.
• Document diagnoses and plan of care.
• Coordinate discharge planning and patient/family education.
• Communicate with the care team to ensure continuity of care.
• Provide evidence-based, age-appropriate nursing care.
