Job summary

Northwestern Medicine Palos Hospital is seeking a Registered Nurse for Transitional Care (Nights). The role centers on caring for patients boarding in the ER awaiting inpatient beds, delivering comprehensive nursing care and collaborating with inpatient teams to support discharge planning.

Required: BSN and IL RN license, BLS; ER/Telemetry or Med-Surg background preferred; ADN with BSN in progress acceptable within 3 years.

Qualifications

• BSN required and Illinois RN license current.
• ADN acceptable with commitment to obtain BSN within 3 years.
• BLS certification required; ACLS/PALS/ENPC/TNCC preferred for ER and Pediatric ER.

Responsibilities

• Assess and record patient status on admission or point of care.
• Organize health data for accurate, accessible plan of care.
• Deliver nursing care with monitoring, medications, and infusions.
• Collaborate with inpatient teams and assist discharge planning.
• Provide patient/family education and document responses.
