Job summary

Beth Israel Lahey Health BIDHC seeks a Team Nurse RN to deliver high-quality patient care in an outpatient primary care setting. Reporting to the Practice Manager and Senior Director of Nursing, this role provides clinical support to Medical Assistants and drives team-based care for BIDHC patients.

Responsibilities include coordinating care, preventive and chronic disease management, patient education, and ensuring PHI safety.

Qualifications

• Graduate from an accredited Nursing Program required.
• License Registered Nurse in Massachusetts required and CPR/BLS certification required.
• 1–3 years related work experience required; new graduates considered.

Responsibilities

• Coordinate and provide high-quality direct patient care in an outpatient primary care setting.
• Educate and train Medical Assistants and patients on preventive, chronic, and episodic care.
• Collaborate with primary care providers to coordinate patient-centered care and transitions between care levels.
• Perform nursing activities and document in the Electronic Health Record in a timely manner.
• Monitor and follow up on abnormal test results and coordinate referrals as needed.
• Oversee inventory of clinical supplies and ensure compliance with policies.
