Job summary

Beth Israel Lahey Health is seeking a Team Nurse RN for our Middleboro Primary Care practice. The role provides direct patient care in an outpatient family medicine setting, coordinating with providers and the MA team to deliver high-quality care.

Responsibilities include AWV/CCM visits, INR management, vaccines, and education, with supervision of MAs and staff. 2+? years of RN experience preferred; Massachusetts license required.

Qualifications

• Graduate from an accredited Nursing Program.
• Massachusetts RN license required.
• Bachelor's degree preferred.
• CPR certification and strong communication skills.

Responsibilities

• Coordinate and manage outpatient nursing care across the continuum.
• Provide direct patient care and education in a team-based primary care setting.
• Perform nursing visits including AWVs and CCM as appropriate.
• INR management and education for Coumadin patients.
• Administer vaccines and medications per orders and policy guidelines.
• Mentor and supervise MA staff and students; ensure timely documentation in EHR.
