Job summary

Beth Israel Lahey Health is seeking a Team Nurse RN to deliver high-quality patient care in an outpatient primary care setting. You will coordinate clinical management for a broad population and serve as a clinical resource for MA staff.

Reporting to the Practice Manager and Senior Director of Nursing, you will educate and train MAs, support transitions between care levels, and counsel patients on health improvements. Strong communication and teamwork are essential.

Qualifications

• Graduate from an accredited Nursing Program; Bachelor’s degree preferred.
• License Registered Nurse in Massachusetts required and BLS/CPR certification.
• 1–3 years related work experience; new graduates considered compatible.
• Strong interpersonal and patient education skills.
• Advanced Microsoft Office applications knowledge.

Responsibilities

• Coordinate and facilitate quality clinical management in outpatient care.
• Collaborate with providers for preventive and chronic care management.
• Perform nursing visits including AWVs and CCM.
• Provide telephonic triage and patient education.
• Oversee inventory and training of MAs and staff.
• Document patient care in EHR and coordinate tests/referrals.
