Qualifications

• Bachelor’s degree in health/human services or related field.
• Minimum 3–5 years of experience in case management or supports planning, preferably with supervisory or lead responsibilities.
• Strong oral and written communication skills with diverse populations.
• Proficiency in Microsoft Office Suite (Word, Excel, Access, Outlook, PowerPoint) and ability to conduct computer-based research.
• Valid Maryland driver’s license and reliable transportation.
• Ability to manage schedules, delegate tasks, and work with minimal supervision.

Responsibilities

• Supervise, mentor, and evaluate a team of Supports Planners/Case Managers, assign caseloads, monitor performance.
• Oversee client needs assessments and development of person-centered Plans of Service.
• Carry a limited caseload including telephonic contacts and on-site visits to support transitions to community independence.
• Coordinate with Social Workers, nurses, and providers to resolve complex needs.
• Ensure accurate documentation and adherence to billing and reporting standards.
• Provide remote training, resources, and oversight to the team.
