Job summary

BLH Nursing is seeking a passionate Supports Planner/Case Manager to provide care coordination for community clients. You’ll assess needs, plan services, and act as the primary contact, with remote training and tools provided to support your role.

Responsibilities include telephonic coordination, home visits, and collaboration with Medicaid providers to ensure service delivery. Bachelor’s degree and strong communication skills are required.

Qualifications

• Bachelor’s degree in health/human services or related field.
• Strong written and verbal communication skills.
• Ability to research using computer and online sources.
• Proficiency in Microsoft Office, Excel, and PowerPoint.
• Ability to write routine reports and correspondence.
• Ability to communicate with individuals with varied cognitive abilities.

Responsibilities

• Assess clients’ needs, strengths, and support networks to determine goals.
• Serve as primary contact with clients and coordinate care.
• Conduct monthly phone calls and home visits; quarterly visits.
• Assist individuals transitioning from facilities to community settings.
• Coordinate meetings with individuals, families, and service providers.
• Maintain documentation in state LTSS system and agency records.
• Collaborate with Local Health Departments and MD Health to enroll clients in programs.
