Job summary

BLH Nursing in Maryland seeks a Team Manager to supervise a team of Supports Planners/Case Managers and manage a limited caseload. The role emphasizes remote training, resource provision, and oversight to ensure high-quality, person-centered service delivery.

The manager will oversee client needs assessments, plans of service, telephonic and on-site care coordination, and collaboration with Social Workers and community providers.

Qualifications

• Bachelor’s degree in health/human services or related field.
• 3–5 years of experience in case management or related field, preferably with supervisory responsibilities.
• Strong oral and written communication; ability to build professional relationships.
• Proficiency with Microsoft Office and ability to review complex documents.

Responsibilities

• Supervise, mentor, and evaluate a team of Supports Planners/Case Managers.
• Carry a limited caseload and monitor client outcomes.
• Provide remote training, resources, and oversight to the team.
• Ensure compliance with organizational policies, billing guidelines, and Maryland Medicaid rules.
• Coordinate client needs assessments and develop person-centered Plans of Service.
