Responsibilities

• Provide PT interventions directed by a licensed Physical Therapist
• Carry out treatment plans focused on strength, balance, gait, mobility, safety, independence
• Monitor residents’ progress and report changes to the supervising PT
• Participate in care conferences within PTA’s scope of practice
• Serve as liaison among residents, facility staff, and therapy team
• Assist the Director of Rehabilitation with assigned administrative and non-clinical tasks
• Complete Net Health documentation accurately and timely

Oasis Therapy Services is seeking a Team Lead Physical Therapist Assistant to provide one-on-one rehabilitation services and assist with therapy operations in independent living, assisted living, and memory care settings in Litchfield, Minnesota.

Responsibilities

• Provide physical therapy interventions directed by and under the supervision of a licensed Physical Therapist.
• Carry out treatment plans focused on strength, balance, gait, mobility, safety, and independence.
• Monitor residents’ progress and report changes or concerns to the supervising Physical Therapist.
• Participate in care conferences within the PTA’s scope of practice.
• Serve as a liaison among residents, facility staff, and the therapy team.
• Assist the Director of Rehabilitation with assigned administrative and non-clinical managerial tasks.
• Complete accurate and timely Net Health documentation.

Qualifications

• Active and current Minnesota Physical Therapist Assistant license.
• Graduate of a CAPTE-accredited Physical Therapist Assistant program.
• Current CPR certification.
• Strong leadership, communication, organization, and documentation skills.
• Geriatric, independent living, assisted living, memory care, or rehabilitation experience is preferred.
