Job summary

Oasis Therapy Services in Litchfield, MN seeks a Team Lead Certified Occupational Therapy Assistant to provide one-on-one rehabilitation and help manage therapy operations across independent living, assisted living and memory care settings.

As a supervising COTA, you will implement treatment plans, mentor staff, coordinate with the licensed OT, and ensure accurate Net Health documentation, while fostering a supportive, patient-centered environment.

Qualifications

• Active Minnesota COTA license in good standing.
• NBCOT COTA certification in good standing.
• Graduate of an ACOTE-accredited OTA program.
• Current CPR certification.
• Strong leadership, communication, organization and documentation skills.
• Geriatric, independent living or memory care experience preferred.

Responsibilities

• Provide OT interventions delegated by and under supervision of a licensed OT.
• Carry out treatment plans focusing on ADLs, mobility, safety and independence.
• Monitor residents' progress and report changes to the supervising OT.
• Participate in care conferences within COTA scope of practice.
• Serve as liaison among residents, facility staff and the therapy team.
• Assist the Director of Rehabilitation with administrative tasks.
• Complete accurate Net Health documentation.

Oasis Therapy Services is seeking a Team Lead Certified Occupational Therapy Assistant to provide one-on-one rehabilitation services and assist with therapy operations in independent living, assisted living, and memory care settings in Litchfield, Minnesota.

Responsibilities

• Provide occupational therapy interventions delegated by and under the supervision of a licensed Occupational Therapist.
• Carry out treatment plans focused on activities of daily living, functional mobility, safety, and independence.
• Monitor residents' progress and report changes, concerns, or the need for reevaluation to the supervising Occupational Therapist.
• Participate in care conferences within the COTA's scope of practice.
• Serve as a liaison among residents, facility staff, and the therapy team.
• Assist the Director of Rehabilitation with assigned administrative and non-clinical managerial tasks.
• Complete accurate and timely Net Health documentation.

Qualifications

• Active and current Minnesota Occupational Therapy Assistant license.
• Active NBCOT COTA certification in good standing.
• Graduate of an ACOTE-accredited Occupational Therapy Assistant program.
• Current CPR certification.
• Strong leadership, communication, organization, and documentation skills.
• Geriatric, independent living, assisted living, memory care, or rehabilitation experience is preferred.
