Job summary

Oasis Therapy Services is seeking a Team Lead – Certified Occupational Therapy Assistant (COTA) to provide one-on-one rehabilitation in an assisted living setting in Cannon Falls, MN. You will collaborate with the DOR on coordination and care planning, while leading a small therapy team.

The role emphasizes independent work under OT supervision, strong documentation, and direct resident care with a focus on ADLs, mobility, and safety. Part-time hours available with potential for additional hours.

Qualifications

• Active Minnesota OTA license.
• NBCOT COTA certification in good standing.
• Graduated from an accredited OTA program.
• Current CPR certification.
• Strong leadership, communication, organization, and documentation skills.
• Ability to work independently while under OT supervision.
• Geriatric/assisted living or team leadership experience preferred.

Responsibilities

• Provide OT interventions delegated by an OT.
• Carry out individualized intervention plans focusing on ADLs, mobility, safety, and independence.
• Participate in resident care conferences with progress updates.
• Serve as a liaison among residents, staff, and the therapy team.
• Assist the DOR with administrative, coordination, scheduling, and managerial tasks.

Oasis Therapy Services is seeking a Team Lead – Certified Occupational Therapy Assistant (COTA) to provide one-on-one rehabilitation services, participate in care conferences, serve as a liaison with residents and staff, and assist the Director of Rehabilitation (DOR) with some managerial tasks in an assisted living setting in Cannon Falls, Minnesota.

Responsibilities

• Provide occupational therapy interventions delegated by and under the supervision of a licensed Occupational Therapist.
• Carry out individualized intervention plans developed by the supervising Occupational Therapist, focusing on ADLs, mobility, safety, and independence.
• Participate in resident care conferences by sharing observations and progress updates within the scope of the role.
• Serve as a liaison among residents, facility staff, and the therapy team.
• Assist the DOR with some administrative, coordination, scheduling, and managerial tasks.
• Monitor residents’ progress and report changes, concerns, or the need for reevaluation to the supervising Occupational Therapist.
• Complete accurate Net Health documentation and collaborate with the care team.

Qualifications

• Active and current Minnesota Occupational Therapy Assistant license.
• Active NBCOT COTA certification in good standing.
• Graduate of an accredited Occupational Therapy Assistant program.
• Current CPR certification.
• Strong leadership, communication, organization, and documentation skills.
• Ability to work independently while following required Occupational Therapist supervision.
• Geriatric, assisted living, skilled nursing, rehabilitation, care conference, or team leadership experience is preferred.
