Job summary

Qualifications

• Active Minnesota PTA license
• Graduate of a CAPTE-accredited PTA program
• Current CPR certification
• Strong leadership, communication, organization and documentation skills
• Geriatric, independent living, memory care, or rehab experience preferred

Responsibilities

• Provide PT interventions directed by a licensed Physical Therapist
• Carry out treatment plans focused on strength, balance, gait, mobility, safety, independence
• Monitor residents’ progress and report changes to the supervising PT
• Participate in care conferences within PTA’s scope of practice
• Serve as liaison among residents, facility staff, and therapy team
• Assist the Director of Rehabilitation with assigned administrative and non-clinical tasks
• Complete Net Health documentation accurately and timely
