Job summary

Oasis Therapy Services in Cannon Falls, Minnesota, is seeking a Team Lead - Occupational Therapist (OT) to provide one-on-one rehabilitation and lead OT services in an assisted living setting.

The role includes evaluating residents, developing tailored intervention plans, and coordinating care with an interdisciplinary team. You will also assist the Director of Rehabilitation with administrative tasks and supervise OT Assistants as needed.

Qualifications

• Active Minnesota Occupational Therapy license.
• Graduate of an ACOTE accredited OT program.
• Current CPR certification.

Responsibilities

• Direct the resident evaluation and reevaluation process.
• Develop and document individualized occupational therapy intervention plans.
• Provide therapy focused on ADLs, mobility, safety, and independence.
• Monitor residents' progress and modify intervention plans as needed.
• Direct and document discharge planning and recommendations.
• Participate in resident care conferences and coordinate occupational therapy services with the interdisciplinary care team.
• Serve as a liaison among residents, facility staff, and the therapy department.
• Assist the DOR with some administrative, coordination, and managerial tasks.
• Provide and document required clinical supervision of Occupational Therapy Assistants when assigned.
• Educate residents and caregivers on exercises, adaptive techniques, and equipment.
• Complete accurate Net Health documentation and collaborate with the care team.

• Ability to work independently and collaborate effectively with residents, staff, and therapy professionals.
• Geriatric, assisted living, skilled nursing, rehabilitation, care conference, or team leadership experience is preferred.
