Job summary

Oasis Therapy Services in Litchfield, Minnesota, seeks a Team Lead Physical Therapist Assistant to provide one-on-one rehabilitation services in independent living, assisted living, and memory care settings. You will work under a licensed Physical Therapist and liaise with residents, staff, and the rehabilitation team.

The role emphasizes leadership responsibilities, quality documentation, and participation in care conferences with a focus on geriatric rehabilitation.

Qualifications

• Active Minnesota PTA license.
• Graduate of a CAPTE-accredited Physical Therapist Assistant program.
• Current CPR certification.
• Strong leadership, communication, organization, and documentation skills.
• Geriatric, independent living, assisted living, memory care, or rehabilitation experience preferred.

Responsibilities

• Provide physical therapy interventions directed by and under the supervision of a licensed Physical Therapist.
• Carry out treatment plans focused on strength, balance, gait, mobility, safety, and independence.
• Monitor residents' progress and report changes or concerns to the supervising Physical Therapist.
• Participate in care conferences within the PTA's scope of practice.
• Serve as a liaison among residents, facility staff, and the therapy team.
• Assist the Director of Rehabilitation with assigned administrative and non-clinical managerial tasks.
• Complete accurate and timely Net Health documentation.
