Job summary

Oasis Therapy Services in Litchfield, Minnesota seeks an Occupational Therapist to provide one-on-one rehabilitation at Eden Vista Litchfield, a senior living community. This PRN role emphasizes individualized treatments and coordination with the care team.

The therapist will assess residents, design OT plans, and educate residents and caregivers while ensuring documentation in Net Health. Minnesota OT license and NBCOT OTR required.

Qualifications

• Active and current Minnesota OT license.
• NBCOT OTR certification in good standing.
• Graduate of ACOTE-accredited Occupational Therapy program.
• Current CPR certification.
• Strong communication, organization, and documentation skills.
• Geriatric/IL/AL/memory care experience preferred.

Responsibilities

• Evaluate residents and develop individualized occupational therapy plans.
• Provide one-on-one treatment focused on activities of daily living, functional mobility, safety, and independence.
• Monitor residents’ progress and modify treatment plans as needed.
• Complete discharge planning and provide recommendations for continued care.
• Educate residents and caregivers on exercises, adaptive techniques, and equipment.
• Direct and supervise Occupational Therapy Assistants as appropriate.
• Complete accurate Net Health documentation and collaborate with the care team.
