Resources for Human Development (RHD) is seeking a Team Lead in the Ardmore/Bryn Mawr area to supervise direct care staff and coordinate services. You’ll mentor staff, manage schedules, and support individuals to achieve personal goals and independence.

Qualifications

• CPRP Certification required
• Valid Driver's License required
• Bachelor's Degree in a Human Services field or related field required
• Associate Degree with at least three years of relevant experience required
• High School Diploma or GED with at least five years of relevant experience accepted
• Strong communication, leadership and interpersonal skills essential
• Ability to work collaboratively with staff, families, guardians and partners

Responsibilities

• Provide first-line supervision, coaching, mentoring, and performance support to direct care staff
• Organize, schedule, and distribute work assignments among team members
• Communicate program policies, procedures, and expectations to staff
• Maintain a direct service caseload and provide direct support as needed
• Coordinate and oversee medical appointments and follow-up care
• Communicate with nurses, physicians, guardians, family members, and other stakeholders regarding individual needs
• Participate in person-centered planning meetings and support individualized plans
• Ensure documentation, progress notes, and monthly reviews are completed accurately and on time
• Coordinate communication with service coordinators and interdisciplinary teams
• Prepare reports and maintain documentation in compliance with agency and regulatory requirements
• Support quality improvement initiatives and ensure regulatory compliance
• Perform other duties as assigned
