Resources for Human Development (RHD) is seeking a dedicated Team Lead to guide direct care staff and help individuals achieve personal goals. This role emphasizes mentoring, coordination of services, and compassionate, person-centered support in a community-based setting.

Based in Pennsylvania with openings in Bryn Mawr and Ardmore, the Team Lead will supervise staff, coordinate schedules, ensure high-quality documentation, and promote independence for the people we serve.

Qualifications

• CPRP Certification required.
• Valid Driver’s License required.
• Bachelor’s degree in a human services field or equivalent.
• Associate degree with 3+ years of relevant experience.
• High school diploma with 5+ years of relevant experience.

Responsibilities

• Provide first-line supervision, coaching, and performance support to direct care staff.
• Organize, schedule, and distribute work assignments among team members.
• Communicate program policies, procedures, and expectations to staff.
• Maintain a direct service caseload and provide direct support as needed.
• Coordinate medical appointments and follow-up care.
• Communicate with nurses, physicians, guardians, family members, and other stakeholders.
• Participate in person-centered planning meetings and support individualized plans.
• Ensure documentation, progress notes, and monthly reviews are completed on time.
• Coordinate communication with service coordinators and interdisciplinary teams.
• Prepare reports and maintain documentation in compliance with agency and regulatory requirements.
• Support quality improvement initiatives and regulatory compliance.
