Job summary

Moments Hospice seeks a Team Director – Hospice (RN Clinical Manager) to guide an interdisciplinary team of nurses, aides, social workers, chaplains, and volunteers, ensuring high-quality, compliant hospice care.

You’ll mentor clinicians, coordinate with families and physicians, and uphold clinical standards while advancing a patient- and family-centered approach in a supportive leadership role.

Qualifications

• Active RN license in the state where you work.
• Experience leading an interdisciplinary care team in hospice or palliative care.
• Strong knowledge of Medicare hospice regulations and compliance.
• Excellent communication and documentation skills.
• Valid driver's license and reliable vehicle with current insurance.

Responsibilities

• Lead and oversee an interdisciplinary team of nurses, aides, social workers, chaplains, bereavement coordinators, and volunteers.
• Provide clinicians with real-time guidance, mentoring, and clinical problem-solving support.
• Lead interdisciplinary group (IDG) meetings and ensure plans of care are accurate, current, and well documented.
• Conduct clinical record reviews, case reviews, and home visits to ensure quality care.
• Ensure compliance with Medicare hospice regulations and maintain survey readiness.
• Manage staff scheduling while fostering a positive, collaborative team culture.
• Develop and update patient care policies and emergency preparedness procedures.
• Serve as the central point of communication between patients, families, physicians, other providers, and the care team.
• Maximize resource utilization to support efficient, high-quality care.

• Active Registered Nurse (RN) license in the state you work in
• Proficiency with electronic medical records (EMR)
• Leadership or management experience a major plus
• Strong understanding of the hospice philosophy and end-of-life care
• Strong organizational, communication, and critical-thinking skills
• A compassionate, patient- and family-centered approach
• Valid driver's license and reliable vehicle with current insurance
