Job summary

Moments Hospice seeks an experienced RN Clinical Manager/Team Director to lead an interdisciplinary team of nurses, social workers, chaplains, and more. You will ensure holistic, compliant care and coordinate with families and physicians, while driving high-quality outcomes.

Qualifications

• RN license with minimum 3 years healthcare experience, preferably in hospice or related settings.
• Bachelor's degree strongly preferred.
• Leadership or management experience a major plus.
• Valid driver's license (required)

Responsibilities

• Lead and oversee an interdisciplinary team of nurses, aides, social workers, chaplains, and more.
• Ensure adherence to hospice regulations and standards.
• Facilitate communication between medical staff and the care team.
• Lead interdisciplinary meetings, documenting patient needs and care plans.
• Develop and update patient care policies and emergency procedures.
• Manage staff scheduling, performance reviews, and foster a positive work environment.
• Act as the central hub for communication with families, physicians, and other providers.
• Analyze quality metrics and manage a caseload of patients as needed.
• Participate in after-hours and on-call duties to ensure seamless care.
• Maximize resource utilization and promote a collaborative work culture.
