Moments Hospice in Florida is seeking a Team Director – Hospice (RN Clinical Manager) to lead an interdisciplinary team of nurses, aides, social workers, chaplains, bereavement coordinators, and volunteers delivering compassionate, compliant end-of-life care. You will mentor clinicians, coordinate with families and physicians, and uphold high-quality standards across the organization.

Qualifications

• Active RN license in the state you work in.
• Proficiency with electronic medical records (EMR).
• Leadership or management experience a major plus.
• Strong understanding of the hospice philosophy and end-of-life care.
• Strong organizational, communication, and critical-thinking skills.
• Compassionate, patient- and family-centered approach.
• Valid driver’s license and reliable vehicle with current insurance.

Responsabilités

• Lead and oversee an interdisciplinary team of nurses, aides, social workers, chaplains, bereavement coordinators, and volunteers.
• Provide clinicians with real-time guidance, mentoring, and clinical problem-solving support.
• Lead interdisciplinary group (IDG) meetings and ensure plans of care are accurate, current, and well documented.
• Conduct clinical record reviews, case reviews, and home visits as needed to ensure quality care.
• Ensure compliance with Medicare hospice regulations and maintain survey readiness.
• Manage staff scheduling while fostering a positive, collaborative team culture.
• Develop and update patient care policies and emergency preparedness procedures.
• Serve as the central point of communication between patients, families, physicians, other providers, and the care team.
• Maximize resource utilization to support efficient, high-quality care.
