Job summary

Moments Hospice is seeking an RN Case Manager to lead care for our patients in Wisconsin. This field-based role focuses on symptom management, care coordination, and family support, with a Monday–Friday schedule and on-call expectations as needed.

You will collaborate with the care team to deliver compassionate end-of-life care, manage caseloads, and engage with leadership for professional growth and impact.

Qualifications

• RN license required.
• Proficiency with electronic medical records (EMR).
• Strong organizational and communication skills; critical thinking.
• Ability to work independently while delivering high-quality care.
• Compassionate, patient- and family-centered approach.
• Valid driver’s license and reliable vehicle with current insurance.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients’ physical, emotional, and psychosocial needs.
• Develop, implement, and update individualized plan of care in partnership with patients and families.
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures.
• Educate patients and caregivers on disease progression, symptom management, medications, and end-of-life expectations.
• Coordinate care across your caseload and serve as the primary liaison among patients, families, physicians, and the care team.
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care.

RN Case Manager – Hospice

Pay: $75,000 – $90,000 per year

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:00 PM
