Moments Hospice in Illinois seeks a Team Director – Hospice (RN Clinical Manager) to lead an interdisciplinary team of clinicians and volunteers, guiding high-quality, compliant hospice care. You will mentor staff, coordinate care with families and providers, and ensure efficient operations while supporting the organization’s growth and leadership access.

The role emphasizes clinical leadership with a focus on patient-centered care, regulatory compliance, and ongoing training.

Qualifications

• Active RN license in the state where you work.
• Strong leadership and team management experience preferred.
• Excellent organizational and communication skills; ability to mentor clinicians.

Responsibilities

• Lead an interdisciplinary team of nurses, aides, social workers, chaplains, bereavement coordinators, and volunteers.
• Provide clinicians with real-time guidance, mentoring, and problem-solving support.
• Lead IDG meetings and ensure care plans are accurate and well documented.
• Conduct chart reviews, case reviews, and home visits to ensure quality, compliant care.
• Ensure Medicare hospice regulatory compliance and survey readiness.
• Manage staff scheduling and foster a positive team culture.
• Develop and update patient care policies and emergency procedures.
• Serve as central communication link between patients, families, physicians, and the care team.
• Maximize resource utilization for high-quality care.

• Provide clinicians with real-time guidance, mentoring, and clinical problem-solving support
• Lead interdisciplinary group (IDG) meetings and ensure plans of care are accurate, current, and well documented
• Conduct clinical record reviews, case reviews, and home visits as needed to ensure quality care
• Ensure compliance with Medicare hospice regulations and maintain survey readiness
• Manage staff scheduling while fostering a positive, collaborative team culture
• Develop and update patient care policies and emergency preparedness procedures
• Serve as the central point of communication between patients, families, physicians, other providers, and the care team
• Maximize resource utilization to support efficient, high-quality care

Qualifications

• Active Registered Nurse (RN) license in the state you work in
• Proficiency with electronic medical records (EMR)
• Leadership or management experience a major plus
• Strong understanding of the hospice philosophy and end-of-life care
• Strong organizational, communication, and critical-thinking skills
• A compassionate, patient- and family-centered approach
• Valid driver's license and reliable vehicle with current insurance
